Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
● You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
● Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
● If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
● Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.
Insurance
If you choose to use your health insurance to pay for therapy services, it is highly recommended and very important that you contact your insurance provider’s customer service department and make sure they fully understand their mental health/behavioral health benefits, including but not limited to: deductible, copay, co-insurance (if applicable), and telehealth visits. Each insurance policy may have different guidelines. The client is responsible for services not covered by their insurance, for example, if the deductible is not met, the claim is denied, or insurance coverage becomes inactive or lapses.
Lakefront Mental Health PLLC will submit insurance claims on behalf of the client for the following insurance plans:
Aetna
Blue Cross Blue Shield
United Health Care
OUT-OF-NETWORK reimbursement
Depending on the insurance provider, clients may have the option to pay cash for therapy services and then self-submit claims to be reimbursed for out-of-network provider expenses by their insurance company. It is the client’s responsibility to contact their health insurance provider, confirm coverage, and secure any and all authorizations. It may be helpful to ask questions like:
Do I have out-of-network coverage?
What is the deductible?
Is there coinsurance or copay required?
Is there a maximum number of sessions per year allowed? If so, how many?
Does my plan cover the following CPT codes for outpatient psychotherapy sessions without requiring a prior authorization? If so, what is the reimbursement rate.
Therapy Session Fees
